It is with a heavy heart that I read of the demise of CFC (NS) Pang whilst on reservist training in New Zealand.

I am writing this article to provide some clarity and my opinion on the circumstances of this tragic occurrence.

I am writing this article as a concerned Singaporean, National Serviceman (NSman) and a person with experience in the field of Workplace Health & Safety (WHS) in civilian industries. CFC (NS) Pang served with Singapore Armed Forces (SAF) as an Armament Technician; to put it simply, he services and repairs weapons. In this case, the Singapore Self-Propelled Howitzer (SSPM) which is an artillery weapon mounted onto an armoured vehicle (photo above).

CFC (NS) Pang’s vocation as an Armament Technician was in the capacity of an Operationally Ready National Serviceman or commonly known as an NSman or reservist soldier. Singaporean males serve 2 years of Full-Time National Service (NSF) at around the age of 18 years old and upon completion of this term become part of a reservist force called NSmen. NSmen are called-up for periodic training and exercises to maintain their skills as soldiers. This was the case for CFC (NS) Pang, he was an actor and a reservist soldier on exercise in New Zealand.

The SAF reports that there were two other soldiers with CFC (NS) Pang when this incident occurred. It is in my opinion that these two other soldiers were the Gunner and Commander (usually the rank of Third Sergeant).

The reports stipulate that CFC (NS) Pang was under the gun barrel when the incident occurred. It is obvious that the artillery gun was not functioning as it should, hence the need for CFC (NS) Pang to rectify the issue; that was his vocation and job.

The gun barrel was in a raised position and it was report by the SAF that CFC (NS) Pang sustained serious injuries when the gun barrel was lowered. It is in my opinion that CFC (NS) sustained crush injuries to his chest and abdomen as a direct result of the gun barrel being lowered onto him while he was servicing the equipment.

The gun barrel may have been unknowingly lowered by another soldier onto CFC (NS) Pang or CFC (NS) Pang may have accidentally caused it to drop onto himself e.g. rupture of the hydraulic lines.

In safety conscious civilian industries, when a technician is servicing or repairing a piece of equipment, one of the most important tasks is to ‘de-energise’ the equipment. To ‘de-energise’ an equipment essentially means to ensure that no energy is running through it e.g. electrical, hydraulic, pneumatic, etc.

An example of ‘de-energising’ in the civilian world is of a technician repairing hydraulic hoses inside the engine of a Boeing 747 aeroplane. Procedures are put in place by reputable airlines to ensure that the engine cannot be started from the cockpit as there is a technician in the engine, this could simply be in the form of ‘locking out’ the ignition switch and ‘tagging’ it as under maintenance. This is commonly called ‘lock-out & tag-out’.

Another practice in civilian industries is to secure suspended loads with solid anchors. A simple example is vehicle mechanics repairing faults under a car. No reputable workshop would allow their mechanics to work under a car that is held up by a hydraulic jack or hoist only. Leaking hydraulic fluid from the jack or hoist would cause the car to crush onto the mechanic. To overcome this issue, vehicle stands are used to securely hold up cars after jacking and the hoist pillars are secured with locking pins.

As a soldier in the SAF myself, I have seen numerous instances where the designated Safety Officer for a training exercise has little to no training and/or experience to undertake this important role. In my experience, the Safety Officer role is usually tasked to a junior officer of Second Lieutenant (2LT) or Lieutenant (LTA) rank who just ‘copies and pastes’ the Safety documentation from previous sessions. These safety dockets address simple issues like hydration, rest, evacuation routes, medics, etc. They do not address technical issues like I’ve mentioned before. I do not blame the young Safety Officers for this, he or she does not know any better. The senior commanders at battalion, brigade and division levels and upwards need to take accountability. This poor ‘culture’ is the root cause.

Commander’s experience plays a big part as well. Personally, I felt the safest in the SAF when I was undergoing training with the former School of Infantry Specialists (SISPEC). SISPEC was a training school where the junior commanders were Non-Commissioned Warrant Officers with at least 10 years’ experience in the field. Although the safety documentation was no different, these Warrant Officers had the experience and ‘common sense’ to ensure the safety of their men.

I urge my fellow Singaporeans to demand for an independent inquiry into the circumstances that lead to CFC (NS) Pang’s death. A panel of experts with varying backgrounds in the military, corporate cultures, incident investigations, root-cause-analysis and WHS needs to address this matter. To be truly transparent, the panel needs to be made up of both Singaporean and foreign experts who have no links to the SAF and Singapore government.

To CFC (NS) Pang’s family and loved ones, I understand that no inquires, reports, analysis will ever bring Aloysius back. I urge you to be strong during this difficult time and not fall for the SAF’s false compassion. You would have received numerous calls from very senior SAF Commanders and ministers; they’ve even ‘gone out of their way’ to fly medical expertise to attend to Aloysius in New Zealand. The key question you need to ask yourselves is; where were they before all of this? Was enough done to ensure he was in a safe working environment? This independent inquiry can only eventuate with your support and blessing.

    It is not an accident if it was preventable. The risk was always there. Who was lowering the barrel? Why could this not be stopped in time to prevent crushing Aloysius?

    Defense Minister needs to held accountable for this tragedy. He should resign or be sacked. His statement from an earlier death that it is not possible to have 100% safe training likely send the wrong message to those at Mindef. They stopped taking MAXIMUM effort to eliminate or at least minimise risks in military training.

    The last “accident” that involved the death of a serviceman was when an armoured vehicle backed into a Land Rover and crushing the driver. Why was this not preventable?

    Too many death at Mindef. This is another compelling reason for regime change.

Never mind he is lowly educated but such person with practical experience is most safety conscience. Scholars will never understand this.     Likewise, you won’t post a person with car repair experience to be a medical doctor. This is where the top man is sotong about. Do you expect a surgeon to know all these? Please resign.

    Those in SAF who make decisions to post non-technical personnel to do heavy technical maintenance works are the first in line to be held responsible for causing accidents in waiting. Everyone knows there is a department on profiling, especially to identify white horses and taking very good care of them. Don’t tell us there is none looking into this area?

    The whole SAF needs to be revamped. All the top Generals are changing hands too fast. Before one gets to settle into the system, he is posted out to join PAP, new Chief of Staff comes in. The new chief starts all over, trying to outdo the previous one by not using his plans, by implementing his own plans. Before he gets to settle down again, this boy becomes Minister talking to a whole room of businessmen with decades of business experiences. Teaching them how to kee chiu and do business.

    Such a fast pace promotion for this group causes the whole system to go down. Parents must think hard about your sons’ lives and safety in such a disorganized organization. Vote for change, vote for your sons’ safety. Change government. Change one who knows mee siam NEVER has ham.

    Spot on- my condolences to the bereaved family.

    It’s time to seriously send those soldiers who are required to implement and enforce safety through a proper safety course conducted by the MoM or safety consultants. It’s not expensive and will ensure that Safety Officers have the requisite awareness, knowledge, skills and tools to do their job properly.

    I have been a NSmen also and it is appalling to see unqualified soldiers taking on the role of Safety Officer. I hope that our SAF will seriously consider equipping their Safety Officers with the knowledge and skills to do their job the correct way.

    Just to share.

    When I was a recruit, we were told to bring some items out from a Store. I was told to go to a room and bring some stuff down from the top of a rack. There was a ladder and being tired from a long day of training, I climbed a ladder and was almost reaching the top when suddenly a strong hand pulled me and prevented me from going up further. The Storeman chided me and pointed to the big ceiling fan swirling at a fast speed just above me. He saved my life.

    I was very blessed as there was actually no one in the room when I went inside and suddenly this Storeman came in, just in the nick of time, to save me.

    The SAF, rightly or not, is not under the WSH Act of the MoM and they are not subject to safety inspections or audits by the MoM. Our lives depend on the common sense and alertness of ourselves and our fellow servicemen, but this is not adequate. A process of ensuring safety should be instituted – one that is able to identify the hazards caused by unsafe act or unsafe environment and devise mitigation measures to address these hazards, should be in place. There should also be a requirement for regular safety inspections and audits with a view to continuously monitor the effectiveness of the safety regime, and this is in line with the practices of ISO 9001 which many Units have already attained.

    More so, as NSmen suffer fatigue.

    I no longer know what patriotism is. I don’t see what there is for me to fight or to die for now. So that a small group of people who feel entitled to privileges using oppressive rule can live opulent lives at our expense, while the ‘lesser mortals’ only give and don’t deserve anything in return?

    I fulfill my NS obligations because I am compelled to, not because I feel those thugs and scum abusing us deserved to be protected.

    Your lives are cheap, safety officer is just another title without a role. Even the payout, if lucky to have any, is very cheap.
    Dafts are not allowed to sue our Army to seek out justice through our PAP court, allowing the Army to escape any responsibility and accountability.

    It is time to review the role of reservist soldiers. With military equipment getting more complex, it is tall order to expect our boys to hit the road running after spending most of their days doing non-army work. If protecting Singapore is really so important, then have a military staffed by regulars or hire professionals. If MINDEF has the financial bandwidth to buy fancy aircrafts, they have the money to hire the pros.

    this is to prevent the wiperhand, without the rubber, from springing back accidentally and cracking the windscreen.

    so it is SAF’s responsibility to ensure when someone is working on the gun system, that such safety feature is in place.

    we listened to the briefing by SAF yesterday. the colonel Lj said, there are commands, verbal, but this is talking typical SAF Lj. being a mechanical system, and heavy, verbal is NOT enough. there must be fail safe safety.

  • SAF training safety is a joke:

    SAF’s safety records are a disgrace. Their instructors don’t care a hoot about trainee’s safety.
    During the late 1970′s several NS boys drowned during a swimming lesson.
    The instructors simply throw the boys (all non-swimmers) into the deep end of the pool and expect them to find a way to stay afloat. Several drowned. An inquiry was held but it was all hogwash.
    The swimming instructors ignored an important basic rule in teaching swimming. It is to teach the trainees how to breath in water before you allow them in the pool.
    Selection of swimming instructors is absurd. Anyone who knows how to swim can become an intructor.

    This ‘no accountability’ and ‘no blame’ culture starts at the very top, none other than mai hum.

    SAF training safety is a joke:
    SAF’s safety records are a disgrace. Their instructors don’t care a hoot about trainee’s safety.
    During the late 1970′s several NS boys drowned during a swimming lesson.
    The instructors simply throw the boys (all non-swimmers) into the deep end of the pool and expect them to find a way to stay afloat. Several drowned. An inquiry was held but it was all hogwash.
    The swimming instructors ignored an important basic rule in teaching swimming. It is to teach the trainees how to breath in water before you allow them in the pool.
    Selection of swimming instructors is absurd. Anyone who knows how to swim can become an intructor.

    The parents of the NS men who drowned during the training incident in late 1970s should have sued SAF and demand adequate compensation. I know no amount of compensation make make up for the loss of a dear one but at least it does something for the safety of future batches of NSmen.

    The SAF need to incalculate a change in mindset in their commanders and subordinates. Currently, their mind-set is that SAFETY PROCEDURES are for sissies and kia-su’s.
    They think it is gung-ho to ignore SAFETY PROCEDURES.

    It is one thing to die defending your country. But when a young man loses his life because of some stupid idiot instructor in the SAF who think safety procedures is a waste of time is INEXCUSABLE. I have completed all my reservist training in all kinds of SAF units and there are lots of SAF instructors who don’t care a shit about safety. These “Tan Chiat” soldiers only know how to go to the mess and drink and get drunk. I seen them at SAFRA centres everywhere drinking until dead drunk, filling their fat stomachs with DUTY FREE beer paid by tax payers.
