    Some more quality is poor cannot compare with private experienced doc and must wait for at least 1 hr for fake subsidies. WTF.

    aiyoh once an organization gets too big, a lot of fatty & recurring costs have to be spent on co-ordinating the different depts and maintaining the whole organization lar.

    And scrutiny of it tends to get lost as there is no one person fully responsible / accountable (ah kong funds any way) as he or she may get rotated / promoted / moved to another dept within the organization and even the head or ministar could say “need to pay more if you want a MORE reliable service”.

    Have you observed assistants or concierge (in hospital or poly) guiding patients even to take queue numbers or going to the right level or room. water dispensers or toilets that have to be cleaned (cost of utilities) ?

    Then there is a huge professional team of accounts and finance and perhaps internal audit. or very expensive medical machines which may be bought but underutilized which may involve cross departmental subsidy ?

    and upgrades here and there (economic stimulus or capex that needs to be spent ???) to give the organization a more fanciful (swanky look) and spending on new furnishing or rearrangement as though the space can become bigger giving you a new look (to take away the monotony of boring same look) ????

    These added costs may not be necessary in a GP as their smallness has it relative cost advantage – e.g one doctor with one or two assistants / nurses will “cover the mountain and cover the sea”.

    Let’s say I try to sell my mao san wang durian to somebody at $50 per kg. Then somebody will say wah why so expensive? Then I will tell somebody $50 per kg already being heavily subsidized by emperor son sitting at the top of the durian tree, Lun Lun you also need to buy from me lor.. Good business isn’t it?

    I am a PG, I got my medication in JB which is cheaper than what I got from SG after subsidies.

    Subsidies are not really taking money from you.
    Its actually a way of Claiming that money was
    given to you, to help you. Actually No money was
    given to you.
    When the durian man tells you you can have the
    $10 pack of durian for $7, you think he’s given
    you a discount. He hasn’t. He only quoted you a
    higher price, so it could Look like he gave you
    a discount.

    Subsidies & fees in Polyclinic aren’t representative of market pricing in the private medical practice. This is overinflating govt revenues in medical services which boosts GDP growth numbers. Hit KPI targets, what happens next?
    Also, don’t forget that you’re seeing multiple & anonymous doctors over a lifetime of medical consultation at the Polyclinic and not a single medical practitioner who is intimately aware of your medical conditions & your medical history. You’re nothing but a medical guinea pig to the system. Medical data on patients to Pharmaceutical companies are a valuable information resource. Why do you think that you go for a battery of tests prescribed by the Polyclinic doctors but the doctors don’t even explained unless you prod them for test results & explanations. Most of the time, they think you a fool if you keep quiet!
    Also the Polyclinic like to prescribe mostly generic drugs made from India & other cheap manufacturers. There had been lots of recalls on Indian generic drugs due to hygiene & contamination factors.
    Sad to say this, JB pharmacy sells better prescription drugs from reputed drug companies unlike the polyclinics. And the polyclinic like to prescribe a lot of medication to patients. A number of generic drugs they prescribed were old drugs having gone out of patent protection instead of new ones which were newly introduced. This is the experience from a few users of the polyclinic system after they patronised JB pharmacy & were given a wake-up call!

    The medical costs showing the consultation fees &:govt subsidies at polyclinics maybe on paper only with no transfer of subsidies to the NHG,SH & NUHS groups managing the respective polyclinics from the govt.

    It is not a subsidy but a discount on the medical fees compared vs pte clinics’ medical fees ie, a discount of $6.80.

    We maybe paying a higher fee than the actual medical fee at polyclinics as the operating cost is much lower because of economies of scale.

    Instead of getting subsidy, we maybe paying the marked-up fee of $13.20 (the actual cost maybe $6.60) & the polyclinics make a 100% profit!

    Mind you, govt subsidies are designed to lower the out-of-pocket costs for medical treatments & not to profit the polyclinics.

    How will we know?

    Convoluted method of Accounting treatment by the MIWs. Countries like Taiwan & Australia fully subsidized Medical Consultation for citizens. The pharmaceuticals are part subsidized under Pharmaceutical Act in Australia for approved drugs because certain drugs can be very costly. Nevertheless, the medical system is clear & transparent to users of medical services.
    But S’pore medical system is configured as Corporate Medicine eg for profit basis. But for political reasons (vote buying) & for eternal gratefulness from older citizens who think that PAP loves them, the subsidy mechanism was created. But the medical cost is jacked high first then offsetted by subsidy figure. Then the balance is paid through Medisave that takes the pain away from settling in cash. But the user is no closer to understanding their true medical costs.
    The Congested Polyclinics with long waiting times were like a conveyor belt mechanism using fresh medical graduates (houseman) or 3rd world doctors (working to get medical equivalency) – each doctor have to see a quota of patients every day (maximizing revenues & reducing doctor’s costs by high turnover of patients). Serious medical cases have to have a referral letter from a polyclinic doctor to get Public Hospitalisation rates & admittance, appointment dates from govt hospitals. From here you can see how much money the Ministry of Health generates from the citizens & unwitting foreigners from Polyclinics. Medicine is big business for the govt ie. why private practitioners (GPs) are having a hard time competing. Like NTUC Supermarkets hollowing out the small wet market stall holders! But the trade-off is quantity over quality! Thing falling between 2 chairs because of the need to make money & quickly ie. Quality of Care absent from the system. misdiagnosis, failure to detect problems, etc.
    KPIs probably based on set targets (operating ratios & industry benchmarks, etc) & percentage figures which probably takes the Topline numbers (gross revenue figures before subsidy, etc).
    GDP growth only calculates Topline figure (nominal currency transactions in aggregate); doesn’t figure in terms of outcomes for accurate ROI measurements like general productivity (as in public civil service) nor profit measures (as in corporations).
    Understand this, one can navigate the treacherous water of Corporate Medicine! The system doesn’t work for you, you’re just a digit!
