Not for profit hospital

The Ministry of Health wants to set up a not-for-profit hospital.

I think that this is a bad idea. It does not address the underlying problem - which is to keep the health care cost affordable.

I prefer the following approach:

a) The ministry should set standard fees for the common treatments, and these should apply to all hospitals that wishes to treat public patients.
As the ministry has the data of the cost of large number of cases for each type of illness, it will be able to set the standard fees that are adequate for the doctors and hospitals, and fair to the patients.

b) The ministry should set a cap on the amount that the public patient has to pay for treatment and should pay for all expenses in excess of th cap.
The public patient or their family is not able to judge whether the treatment is necessary and is fair. This question is important for expensive treatment in "end of life" situations. If the government take over the excess expenses, the ministry can set up an impartial panel of medical experts to determine the limit of the treatment. There is no point in providing expensive treatment that prolong life, without any reasonable prospect of recovery. The family should not be asked to make this judgment.

c) All public hospitals, including the "restructured hospitals" should be operated on a not-for-profit basis.

    Not too long ago our tax payer funded PUBLIC hospitals were used for medical tourism. Then kena exposed faster quickly go say that foreigners not allowed at PUBLIC hospitals. In the first place who was the idiot/idiots who allowed PUBLIC hospitals to be used for medical tourism hmm?

    And this so called “not for profit” does not make sense. It gives the impression that the current PUBLIC hospitals that we have now are purely for profit. Why the need for “not for profit” in the first place if that isn’t the aim of current PUBLIC hospitals? Can’t even wayang right these days ah PAP? Your acting skills have certainly gone downhill rapidly compared to the past!

    Hahaha…..when you inflate the population artificially in a small country like Singapore, it comes with unforeseen costs.     Response: families should have the rights to decide for their love ones based on their wishes either to die comfortably with medical help or die with dignity. I don’t see treatment is to prolong life but is a final respect given to the love ones.

    Response: I thought all public hospitals in the 3 clusters are already operated on a not-for-profit basis?

    Just wondering if the new hospital is meant to be private, so by tapping on the present resources of private hospitals and based on economy of scale, probably one-stop service might be cost efficient?

    Maybe it’s time for some private hospitals’ investors to do a good cause since they have made so much profits during pandemic.

    NTUC supposed to be a Union Coperative. A non making organisation but went into Tac services. NTUC supposed to churn money for its Union members. But went further into investment. Today, things are not competitive as Sheng Siong. Even go into lucky draws and media broadcast on variety shows. Giving back to their customers .
    It is sad that since NTUC supermarkets and taxi services are losing money or making profit comparison to Sheng Siong. Are NTUC giving concession in rental in heartland. Now going into Food Court business..
    why go into business that the NTUC coperative are not familiar with. Food for the thoughts.

  • I think if the policies for locals are sound, there would be no need for non-profit hospitals at all, because the coverage would be adequate.

    I heard Malaysia and even China has very good safety net for medical, maybe can study and learn from them?

    For example in China, there is a basic medical coverage (医保) of RMB 360 per year for all hospitalisation (maternal and some dental excepted) of up to 80%. Then there is an extra RMB 99 per year extra coverage (市保) for hospitalisation expenses, above and beyond the basic medical coverage, subject to a maximum of RMB 5 million.

    As an example:

    If someone is hospitalised for cancer and requires surgery and the surgery costs RMB 1,000,000, the basic medical coverage would cover RMB 700,000 (@70% depending on grade of hospital, say top hospital).

    The balance of RMB 300,000 would then be required to be paid by the patient.

    However, with the extra coverage, the patient would only need to fork out RMB 15,000 (免赔) and the extra insurance coverage would take care of the rest of RMB 285,000, up to a maximum of RMB 5 mllion.

    I am currently covered by both and it does take a huge load off my mind as O do not need to worry about huge medical expenses should I kenna a relapse. My expenses in any case is limited to RMB 15,000.

    I heard in Sinkieland there is a $8 plan that has been proven by a millister, but until today I still cannot find where to subscribe.

    Anyone in the know? lol
